Nothing is charged on this page. The payment button does not even appear until your answers are in. We would rather find out the A$490 pack is wrong for you before you have paid for it than after. The form is mostly dropdowns and it asks about your real practice, not your ideal practice: how often you actually supervise, how many evacuation drills you actually ran.

That question style is deliberate. Documentation that does not match how a service actually runs is treated as a risk to the integrity of the audit, and is one of the weaknesses auditors most often cite. Answers built from your real frequencies are the opposite of a template.

You've bought the paperwork and the plan. The decision belongs to an independent auditor and the NDIS Commission. That independence is precisely what makes a certification worth having. Anyone who tells you they can guarantee the outcome is telling you something they cannot deliver.

Answer 11 of 26 · Incident management
The portal asks: How do you identify, manage, report and learn from incidents?

Any worker who witnesses or learns of an incident makes the participant safe first, then reports it to the on-call Service Manager immediately and completes an Incident Report before the end of their shift. On-call is staffed twenty-four hours at Bright Living Supports and the number is displayed in every house.

Every incident is entered in the Incident Register. Five categories must be notified to the NDIS Commission within twenty-four hours: death, serious injury, abuse or neglect, unlawful sexual or physical contact, and sexual misconduct. A detailed follow-up report is then provided within five business days of that notification. The unauthorised use of a restrictive practice sits in a separate class and is notified within five business days. Both sets of categories and both timeframes are printed on the Incident Report form so a worker does not have to recall them at 2am.

Each incident is investigated proportionately to its seriousness. The investigation records what happened, the root cause, what was done, and what will change. The participant and, with consent, their family are told the outcome.

Incident trends by type, house and time of day are reviewed monthly at governance. Where a trend appears, the resulting change is recorded in the Continuous Improvement Register.

The portal uses myID, a personal digital identity tied to one human being, and the application is a legal attestation in your name. Anyone offering to log in and submit as you is offering something that puts you at risk, not them.

We don't guarantee approval

An independent auditor and the Commission decide. We guarantee the documents, the timing, and a refund if we miss either. A guarantee of approval from a documentation service isn't a guarantee. It's a sales line.

We don't get on the phone

This is a design decision, not a limitation. Every question we answer in writing becomes something we fix for the next person. Calls disappear; written answers compound. You also never get chased by a salesperson.
